Job Description
Area: Bloomfield Nursing and Rehab
Class: T-LPN Shift: 2Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse (LPN/LVN) delivers efficient and effective nursing care while promoting positive clinical outcomes and patient satisfaction. The LPN provides direct patient care, assists in nursing assessments, and supervises care provided by unlicensed staff all while operating within the scope of practice defined by the State Nurse Practice Act.
ResponsibilitiesCollect, report, and document objective and subjective patient data.
Observe patient conditions and promptly report changes to the RN.
Contribute to care planning and assist in developing and implementing individualized interventions.
Administer medications and perform treatments per physician orders.
Accurately document care and communicate pertinent information to the RN and physician.
Supervise and assign duties to unlicensed staff as allowed by state regulations and company policy.
Participate in interdisciplinary team meetings and discharge planning.
Ensure proper shift-to-shift communication and collaboration with other departments.
Maintain compliance with safety, infection control, and confidentiality policies.
Utilize PointClickCare (PCC) and other digital tools to document care accurately.
Stay beyond scheduled shift as needed to meet patient or staffing needs.
Graduate of an approved school for Practical or Vocational Nursing.
Current and active LPN/LVN license in the State of New Mexico.
Minimum 1 year of clinical nursing experience required (rehab or geriatric experience preferred).
Must be able to read, write, and communicate effectively in English.
Current BLS/CPR certification .
Ability to maintain confidentiality, professionalism, and a service-oriented attitude.
Must provide verification of TB/TST per state requirements.
Classification: Grade 15 | FLSA: Nonexempt
Must comply with HIPAA and company compliance standards.
Participates in mandatory training, safety programs, and quality improvement initiatives.
